Multilingual Care Coordination

 
Ebenezer provides both social and healthcare assistance to its clients. Services provided incl medical referrals, scheduling patient appointments, arrangement for non-emergency transportation, client’s health and healthcare system literacy, assessment of client requirements for community resources referrals, help clients apply for health insurance, and food assistance.
With our multilingual Care coordination program, we address all social determinants of health that a client might have.

Hotline

 
Ebenezer Hotline provides a multi-lingual telephone line that clients can use to seek for assistance, advise, or general information on available health, healthcare, and social support services.
We can help you with French, Kinyarwanda, Kirundi, Swahili, Lingala, Kinyamulenge, Spanish, Arabic and English languages.
